Award for Excellence In Hospital Medicine
SHM recognizes that the Hospital Medicine team includes many dedicated professionals in addition to the physician hospitalists. SHM plans to recognize individuals of excellence through its annual Awards of Excellence program.
Criteria
While the overall contributions to hospital medicine are the overriding criteria, some or all of the following elements may be considered in the selection process for this award:
- Innovations or contributions at their hospital or in their hospitalist group
- Leadership
- Outstanding contributions to patient care
- Excellence in education
- Professionalism
- Excellence in practice management
- Organizational activities
All submissions require the following:
- Nominee must be an SHM member, who not a physician
- Nominee may be a physician assistant, nurse practitioner, RN, pharmacist, administrator, case manager or any other non-physician member of SHM
- Completed Nomination Form, addressing the specific criteria that are the basis for evaluation of the nominee being considered
- Nominee's Curriculum Vitae submitted with the Nomination Form
- Two letters of recommendation in support of a nominee (if more than two letters are submitted, nominee will be contacted to specify which should be considered: letter should indicate if confidentiality of content is required
